Continence Assessments
QHACS has a clinical team of nurses who specialise in continence assessments offering a national telehealth service or face to face meetings within South East Queensland, for both NDIS and private clients.
Funding and NDIS for your Continence Assessment and Plan
For one of our clinical nurses to provide continence assessment, reviews, therapy or support, it will be funded under Capacity Building – Improved Daily Living.
Our completed continence assessments are incredibly thorough. Our nurses will conduct an interview with you, follow up and read previous reports and OT reports to confirm any changes, travel to you if required, research your products and compile a intricate consumables report to be used to gain additional funding.
The Comprehensive Health Assessment Program (CHAP) Report
The CHAP is a validated tool to assist the participant, carer, guardians or service providers gather information for a comprehensive health assessment. This tool incurs a license fee and is updated annually.
Our Registered Nurses assist participants and their supporters to gather information and provide it to the GP for review. Our nurses will ask more in detail questions to ensure the GP is able to focus on key areas of concern or change.
In addition to the CHAP, our nurses will provide an additional summary of what was contained in the report and highlight any required needs such as additional support hours or recommendations regarding testing. This can be used in addition to the CHAP to strengthen NDIS plan reviews or changes.
CHAP assessments are generally completed face-to-face but can be completed via telehealth if you are unable to find a provider in your locality.
